(For Doctors / Clinics / Hospitals / Labs / Dentists / Physiotherapists / Ayurvedic / Homeopathic / Panchakarma / Diagnostic Centres and other categories)
By submitting this form and joining the BMS network, you agree to the following Terms & Conditions:
1. Enrollment & Validity
- The service provider agrees to join Bhagyoday Mediservices (BMS) as an empanelled Network Service Provider.
- The empanelment is valid for 3 years from the date of acceptance and can be renewed yearly by mutual consent.
2. Services to BMS Members
- Service Provider shall provide treatment/services as per good medical and business practices.
- Service Provider shall provide quality and safe medical services using hygienic and valid (non-expired) medicines and accessories.
- Services shall follow accepted medical standards and government guidelines.
3. Identification of BMS Members
- Members will show their digital BMS Membership Card in the BMS Mobile App.
- No physical card will be issued.
- Service Provider must verify the card before giving benefits.
- Service Provider must display BMS recognition materials (stickers, poster, standee) provided by BMS.
4. Concession / Discount Policy
A. GENERAL PHYSICIANS / CLINICS
30% concession on:
- Consultation
- In-house medicines
- Dressing / Injection / Insulin
- BP / Diabetes check
- Minor treatments
B. HOSPITALS
30% concession on:
- Room Rent
- ICU & Nursing
- Procedure charges
- Operation charges
- Pathology & Radiology
- ECG / Monitor / Emergency / Delivery
- Other hospital service charges
Note: Concession is not applicable on medicines, oxygen, visiting doctors, blood, specialist doctors & anesthesia charges.
C. DIAGNOSTIC / PATHOLOGY / RADIOLOGY CENTERS
30% concession on all tests, investigations & reports.
5. Referral Obligations
- If further treatment is required (hospitalization, lab test, radiology, specialist), the doctor/clinic must refer the member to BMS Network Providers only.
- Service Provider must inform BMS office within 6 hours of referring a patient.
6. Legal & Professional Responsibility
- Service Provider confirms that all licenses, registrations & approvals required to practice are valid.
- Service Provider declares that he/ she/ staff do not have any criminal complaint affecting medical practice.
- Service Provider shall maintain professional indemnity insurance at their own cost.
7. Confidentiality
- All information shared by BMS or members is confidential.
- The provider shall not disclose any confidential information to outsiders without written consent of BMS.
8. Records & Reporting
- Provider must maintain a simple record of BMS Members treated.
- Provider shall participate in monthly feedback & review, if required by BMS.
9. Media & Branding Restrictions
- Provider must not represent BMS to media without written approval.
- Provider must not misuse or duplicate BMS logo/brand.
- After termination, all use of BMS branding must stop immediately.
10. Payment Terms
- All payments (consultation, tests, hospital charges) are to be collected directly from the member.
- BMS is not responsible for any fee recovery or financial dispute between provider and member.
11. Indemnity & Liability
- BMS does not interfere in treatment or medical decisions.
- BMS is not responsible for outcome of treatment, medical errors, negligence, or staff actions of the service provider.
- Service Provider shall indemnify BMS for any legal or financial liability caused due to provider's actions or violation of law.
12. Termination
- Either party may terminate the empanelment with 30 days written notice.
- Empanelment may be immediately cancelled if:
- Provider violates any BMS Terms
- Fraud, misconduct or malpractice is detected
13. Arbitration & Legal Jurisdiction
- Any dispute shall be resolved through Arbitration under Arbitration & Conciliation Act, 1996.
- Legal jurisdiction shall be Thane District Court, Maharashtra.
14. Important Declaration
- Bhagyoday Mediservices is NOT an Insurance or Mediclaim.
- BMS does not assure cashless or free treatment.
- This is a service-based concession program, not an insurance policy.
- Service Provider must refer to this program only as "BMS Service".
Important: By checking the agreement box and submitting this form, you acknowledge that you have read, understood, and agree to be bound by these Terms and Conditions.
Declaration: "I/We hereby accept the above Terms & Conditions and agree to provide services to BMS Members as per the above guide terms and condition for doctors registration."
